What is Endodontics?

Endodontics is a specialty of dentistry that deals with "root canal treatment" (infections, inflammation, pain in the dental pulp or emergencies).

The term "endodontics" comes from the Greek language, and is formed by joining two words "endo" (inside) and "odont" (tooth), so endodontics is a specialty of dental medicine that deals with "root canal treatment" (infections, inflammation, pain in the dental pulp or emergencies).

Endodontic treatment is necessary when the dental pulp becomes inflamed or infected. 

The most common causes of inflammation or infection are: deep cavities, improper crown fillings or dental trauma. In situations where timely intervention is not provided, the dental pulp will become necrotic; the dead pulp tissue will become infected and the bacteria present in it will be able to advance towards the bone, eventually causing its destruction or even the formation of abscesses.

What are the risks of dental pulp removal?

During the growth and development of the tooth, the pulp plays an essential role, since at this stage it fulfills, in addition to its formative and sensory role, a nutritional role. However, once the tooth reaches maturity, it can survive without the dental pulp, as it will continue to be nourished by the tissues surrounding it. Therefore, removing the pulp of a mature tooth does not pose any major risks in the short or long term.

What are the risks of not removing the dental pulp?

If the inflamed or infected pulp of the tooth is not removed in time, the pathology will worsen, the pulp will die, causing the formation of pulp gangrene, and the hard tissues and supporting tissues will also gradually deteriorate, which will ultimately lead to the loss of the tooth. In addition, there is a risk that the infection will extend to the bone level, causing significant destruction and increasing the risk of complications.

Signs that may suggest the existence of pathologies that require Endodontic treatment

Such signs may include, but are not necessarily limited to: pain, prolonged sensitivity to heat or cold, sensitivity to touch and chewing, tooth discoloration, swelling, tenderness in the lymph nodes as well as in the nearby bone and gum tissue. Stages of endodontic treatment

Any effective treatment requires both establishing a correct diagnosis and monitoring the effectiveness of the therapy. Since, as we have shown above, endodontic treatment is a treatment of the interior of the tooth, that is, of a part that cannot be observed directly, in this case, the only way to establish the diagnosis and monitor the therapy is based on taking at least three dental radiographs, namely:

  • Initial radiography, on the basis of which the diagnosis and therapeutic conduct will be established.
  • Second X-ray, which you will perform during treatment, to monitor the effectiveness of the intervention.
  • Final radiography control, which is carried out at the end of treatment.

Sometimes conventional (2D) radiography is not sufficient for a proper diagnosis. The endodontist needs to see better inside, thus requesting a 3D radiography or a CBCT.

There is a fear among patients regarding the risks involved in repeated dental X-rays. It must be said that the advanced radiological equipment currently used exposes the patient to a minimal dose of radiation, which does not justify such fears.

Duration of endodontic treatment

Although endodontic treatment can be performed in a single visit, for optimal results, in some cases it is preferable to perform it in two separate sessions, several days apart. Depending on the pathology, additional root canal treatment sessions may be necessary, especially when we have major difficulties such as fractured instruments, perforations or large apical infections.

Stage 1 – Elimination of the infection

Once the x-ray is examined, the endodontist administers a local anesthetic, isolates the tooth by placing a protective „sheet” called a “dam” to keep the tooth clean during the procedure, preventing it from being contaminated by microbes and bacteria contained in saliva. Through a hole opened in the crown of the tooth, the pulp is carefully removed from the pulp chamber and all root canals, which will then be cleaned and shaped.

When endodontic treatment is performed in two sessions, the hole in the dental crown can be sealed with a temporary material in a first phase, which will then be replaced with a permanent filling upon completion of the treatment.

Stage 2 – Protecting the tooth

Once the pulp is removed, an empty space remains in the root canals and pulp chamber, which must be sealed and sealed; otherwise, this space would immediately be populated by bacteria. Sealing is performed through a process known as vertical hot condensation, using a non-absorbable and compact biocompatible material, known as gutta-percha, together with an adhesive cement, or sealer. This process ensures a homogeneous obturation, as well as durable results.

Stage 3 – Tooth reconstruction

After the endodontic treatment is completed, the tooth will need to be coronally restored so that it can resume its functionality. The dentist will complete the treatment by making a permanent filling.

Most of the time, however, a tooth that requires endodontic treatment has a compromised crown. In these situations, a simple filling is not enough. The dentist will place a fiberglass post inside the tooth, thus increasing its strength, after which the strength structure and aesthetics of the tooth are restored by metal-ceramic, zircon-ceramic or all-ceramic crowns, inlays (inlay, onlay or table top). Recommendations after endodontic treatment

After a root canal treatment, some sensitivity (pain) may occur, to a lesser or greater extent. The endodontist will prescribe the appropriate medication depending on the case.

It is normal for pain to occur - from simple embarrassment, to sensitivity to pain that only subsides with painkillers or anti-inflammatory medication.

Antibiotics may be prescribed/recommended in certain cases, but it depends a lot on the initial diagnosis and the present clinical situation. The endodontist is the one who can prescribe antibiotics.
